Understanding Polycystic Ovary Syndrome Treatment: How TCM Addresses the Root Cause

Irregular cycles, persistent weight gain on your waistline, breakouts that don’t clear with topical creams, hair growth in places you’d rather not, thinning in others. If you identify with this list, you are likely to be of the 10% of women with polycystic ovary syndrome—a hormonal disorder caused by hormonal imbalance, insulin resistance and excess androgens that distorts ovary function.

Conventional women-centric treatments try to address only the overt symptoms. Use of metformin for insulin resistance. Oral contraceptive pills to suppress elevated androgens. Use of spironolactone for hirsutism. Each treatment only works on one pathway within a complex hormonal cascade, ignoring the role of the other pathways, which renders most women cycling through pills for years without feeling genuine body rebalancing.

Integrating TCM for Complete Rebalancing

To treat PCOS effectively, all three patterns need to be tackled. Restoring kidney essence for HPOaxis regulation. Dissipating dampness to improve metabolism. Good blood circulation for ‘breaking the stasis’. This integrative approach is exactly what you find at Tong Ren Tang, and it’s why women who had to rely on pill treatment for many years find better results when receiving combined TCM treatment. Women who originally came to have acupuncture for menstrual pain often find that not only their pain resolves, but other symptoms of life with PCOS have settled as well, because the patterns overlap so much.

The PCOS Cascade — From Insulin to Infertility

Elevated insulin signals your ovaries to produce excess testosterone and other androgens. High androgens interfere with follicle maturation, leading to anovulation — your ovaries develop multiple small cysts instead of releasing a mature egg each month. Without ovulation, progesterone stays low, periods become irregular or absent, and fertility drops. Meanwhile, insulin resistance promotes abdominal fat storage, which further worsens insulin sensitivity. It’s a self-reinforcing loop.

In Chinese thought, PCOS isn’t looked at in isolation. Instead, TCM recognizes pattern combinations, most commonly kidney deficiency (), phlegm-dampness accumulation (), and blood stasis (). Sure enough, these patterns relate directly back to the biomedical aspects of PCOS—in kidney deficiency, we find HPOxis, ‘hypothalamic-pituitary-ovarian’ dysfunction. In phlegm-dampness, we recognize the metabolic syndrome and insulin resistance. Finally, in blood stasis, there is a similarity with the ovarian micro-environment, where follicles never get a chance to mature enough to ovulate.

The Tong Ren Tang Protocol: Integrating TCM For Pcos in Your Personalized Path

PCOS is a syndrome. It falls on a spectrum. A woman with normal weights and only mild hirsutism and irregular cycles and elevated DHEAS needs very different intervention strategies than someone with severe insulin resistance, morbid obesity, and marks of chronic inflammation like acanthosis nigricans. For this reason every PCOS tongue and pulse diagnosis at Tong Ren Tang begins with a full individualized assessment before we outline specific treatment strategies.

Initial Assessment

Your first appointment (approximately 45 minutes) will include pulse diagnosis over 3 locations on each wrist, tongue diagnosis for dampness, heat, or blood stasis, review of your last automated hormone profile (FSH, LH, AMH, insulin, testosterone, DHEAS and others depending on your personal situation), and an interrogation into your menstrual cycles over the last 6-12 months time period. We encourage you to bring all current lab tests with you as we aim to integrate the TCM pattern diagnosis with the biomedical results.

Acupuncture Session

Acupuncture

1-2 sessions weekly for 8 sessions, then every other week as Hb regulation begins. 35-45 minute sessions. Focus on HPO axis regulation with CV4, SP6, ST36, LR3 and other points based on your pattern. Electroacupuncture in cases of sympathetic adrenomedullary hypertone.

Herbal Medicine Preparation

Herbal Medicine

Individualized herbal formulas — not generic supplements. For kidney deficiency patterns, we often start with a modified Gui Shao Di Huang Tang to nourish kidney yin and regulate the HPO axis. Phlegm-dampness patterns may require Cang Fu Dao Tan Tang to resolve metabolic stagnation. Formulas are adjusted every 2–4 weeks based on your response.

Dietary Therapy Guidelines

Dietary Therapy

TCM dietary guidelines tailored to your pattern type. Blood-sugar stabilizing strategies including specific food pairings and meal timing. Anti-inflammatory foods emphasized. We address both what you eat and when you eat relative to your menstrual cycle — follicular phase foods differ from luteal phase recommendations.

Treatment Timeline

Month 1–2: Foundation

Cycle regulation. Bloating, fatigue, and irregular bleeding diminish. Signs of ki regulation are alleviation of emotional tension. Frequency is reduced to weekly.

Month 3–4: Consolidation

Improvement in symptoms, such as a visible decrease in the skin’s acne lesion count, more even chin hair growth, and regular menses can be seen. Herbal dosing follows and tapers with your progress.

Month 5–6: Maintenance

For fertility patients: Ovarian monitoring and timed treatment cycles for conception. For symptom management: switch to biweekly acupuncture and streamlined herbal tonic support. Many patients sustain the results with monthly tune-ups.

Pcos Natural Treatment vs. Conventional Medication: An Honest Comparison

We aren’t here to tell you to abandon your gynecologist. We’re here to provide a plain, honest depiction of possibilities. Here are the three most prevalent other options, compared on the dimensions of greatest concern to those with PCOS.

Dimension Metformin Birth Control Pills Acupuncture + TCM
Mechanism Insulin sensitizer — reduces hepatic glucose output Suppresses ovulation and lowers androgens via synthetic hormones Regulates HPO axis + improves metabolic balance through multiple pathways
Onset 2–4 weeks for insulin effects 1–3 months for skin/hair improvement 1–3 months for cycle regulation
GI Side Effects ~20% of users; nausea 3x and vomiting 3x higher vs placebo Mood changes, weight gain, DVT risk (rare but serious) Minimal — occasional bruising at needle sites
Fertility Impact Neutral to mildly positive Negative while on medication — suppresses ovulation by design Positive — actively improves ovulation rates
Root Cause Partial — addresses insulin pathway only No — masks symptoms, rebounds on stopping Multi-pathway — endocrine, metabolic, and ovarian
Duration Ongoing indefinitely for most patients Ongoing — symptoms return when stopped Course-based (3–6 months) with taper to maintenance

You’ll be interested to learn that these regimens are often combined – women at our practice frequently receive acupuncture while on metformin or letrozole, and vice versa. Practitioners at our clinic communicate with your reproductive endocrinologist or gynecologist so that treatment is integrative and effective. You’re not required to transition off any medications you’re already taking – we see your great care team with respect and compliance.

Course-Based Treatment, Not Lifetime Medication

Most of these medications need to be administered long-term, indefinitely – if you stop, treatment benefits fade. Traditional Chinese Medicine can be administered in a defined, short-term course, with a maintenance period as needed. Patients tend to find that their symptoms are maintained with less-invasive interventions, such as monthly acupuncture sessions and herbal upkeep strategies, rather than daily pharmaceuticals.

Real Patient Journeys: Managing Symptoms with Herbs & Acupuncture For Pcos

Every case of polycystic ovary syndrome is unique. Coming from the experience of patients treated at Traditional Chinese Medicine clinics, these instances represent the spectrum of PCOS cases and the results achieved in our practice. Patient information is provided under a pseudonym.

Patient Journey — Fertility Focus

Sara was a healthy 28-year-old who had been trying to conceive for two years. Her cycle length varied from 45 to 90 days, and a gynecologist confirmed infrequent ovulation. Conventional infertility medications, including clomiphene and finally metformin, failed to produce results.

She began acupuncture and herbal care while on traditional ovulation induction drugs. Her period length decreased noticeably from 45 days to 38, to 32 days, over 4 months until total ovulation was visible on ultrasound.

She achieved conception without other medications at 6 months. Successful conceptions in PCOS are common, whether on various fertility drugs or with or without traditional Chinese Medicine integrated care.

Patient Journey — Metabolic Recovery

Baseline data

Amira was age 33, with BMI of 31. The results of oral glucose tolerance tests included an A1C of 5.8% (prediabetic level) and evidence of insulin resistance; she was prescribed metformin which she stopped after 6 weeks due to intolerable gastrointestinal side effects (nausea, diarrhea).

Month 1

acupuncture (twice weekly), herbal formulation to clear excess phlegm dampness. Dietary counseling began- minimized sugar and processed foods; introduced spleen-boosting, warming, yin increasing foodstuffs.

Month 2 & 3

Month 2: report of feeling better, less bloated, more energy. First period after 3 months – light but present.
Month 3: down 4 kg. Fasting glucose relieved. Acupuncture frequency reduced to weekly. Amira reported: “I’m feeling great.”

Month 4

repeat A1C: 5.5%. Summary: 8 kg of weight loss. Periodic cycles averaging 35 days in length.

Patient Journey — Hormonal Acne & Cycle Regulation

“3 months on oral medication sorted my hormonal acne and I had return of my periods for the first time in a year. I had refused contraception contraception because I didn’t want to mess around with synthetic hormones- this was the solution I had been looking for.”

Priya (25) came because of drenching cystic acne on the jaw and chin—characteristic of androgen activity. Priya had been amenorrheic for 14 months. Her biochemical picture showed an elevated testosterone and an LH: FSH ratio of 3:1.

Priya improved markedly in three months with a course of biweekly liver qi-regulating/ blood-heat clearing acupuncture combinations and a tailored Chinese herbal prescription. Priya’s menses returned within a 34 day cycle. Six month follow up from her dermatologist showed a significant diminution of inflammed pustules in the absence of contraceptive hormones.

Frequently Asked Questions— PCOS Treatment

Each woman’s presenting pattern differs based on her dominant imbalance – insulin resistance, excess androgens, inflammatory, post-pill and so on. However a multi-pathway treatment plan that combines multiple modalities has provided the best results in research trials: lifestyle modification (diet and exercise) as a base-line with additional targeted intervention. Many women have found that acupuncture + Chinese herbal medicine simultaneously respond to hormonal imbalances and metabolic imbalances in ways that targets of just a single drug cannot. Metformin only targets insulin; birth control only targets androgens. This is exactly what a specific TCM protocol can offer through a multi-approach, multi-targeted pathway. This explains the increased ovulation rates seen in research using 20+ acupuncture treatments.

Yes. Existing peer-reviewed research shows: women with PCOS who had acupuncture showed decreases in circulating levels of testosterone and DHEA-S, clinical improvements in frequency of ovulation, and regulation of menstrual cycles. The mechanism was: stimulation of Δ-endorphin release, affecting the HPO axis, and reduction of over activity of ovarian sympathetic nerves.

Most patients will be required to stay for a period of 3-6 months. The most evidence for cycle regulation should be seen in about 6-8 weeks. If looking to stabilize symptoms (acne, hirsutism, weight) , then this data can be expected to be seen around 3-4 months. Patients that are hoping to become pregnant may wish to extend treatment through conception.

PCOS is a complex disease with genetic, metabolic and environmental factors involved. The basic pathway involves insulin resistant stimulation of excessive ovarian androgen production which inhibits ovary follicle development. Other factors include inflammatory processes, substrate issues (gut health), and the stress hormone cortisol. In traditional Chinese medicine this pathway fits a diagnosis of kidney xu, phlegm dampness encumbrance, and blood stasis – three contributing syndromes that will need to be corrected to sustain long term benefits.

Indeed. Acupuncture has been demonstrated to raise ovulation rates through modulation of gonadotropin secretion and amplification of ovarian blood flow. Herbal formule such as modified Gui Shao Di Huang Tang can be used to ameliorate follicle growth as well as a thickening of the endometrial lining. Many women who have failed Clomiphene or letrozole alone have benefitted from the inclusion of acupuncture. We also work in conjunction with IVF clinics – acupuncture before and after embryo transfer has established efficacy in this adjunct setting.

Many UAE health insurance plans cover acupuncture and TCM services as part of complementary and alternative medicine (CAM) benefits. Coverage varies by insurer and plan level. We issue DHA-compliant receipts and treatment documentation for reimbursement purposes. Contact your insurer to confirm your specific coverage before booking.

Yes – and a lot of our patients do just this. As we mentioned earlier, the two treatments are working in completely different ways (HPO axis regulation compared with insulin sensitization), so can be used together or separately depending on how well each one works for you. Some women report that acupuncture alleviates the gastrointestinal side effects of metformin, but this has not been scientifically studied. If you are already on metformin (or any other medication), your practioner needs to know about this on your first visit so treatment can be planned accordingly.

Yes. They relieve the symptoms, but they don’t correct the abnormal metabolic and hormonal dysfunction that caused the symptoms to develop in the first place. When you go off the pill the symptoms often return, sometimes even more severe. Traditional Chinese medicine offers an alternative to chemicals, working on restoring your body’s corrective ability. For women who prefer not to take synthetic hormones or for those preparing to conceive, the options include acupuncture, herbal remedies and dietary therapy.

History and diagnosis are commonly performed by gynecologists and reproductive endocrinologists. If skin complaints are involved, dermatologists sometimes treat them. Your TCM practitioner who is licensed under the Department of Health and administered a11 TCM TCM practitioners in Hong Kong is authorized to treat you with acupuncture, Chinese herbal medicine and dietary therapy. An integrative approach with your gynecologist would produce the most favorable outcome.

So PCOS isn’t cured – it exists, but the symptoms can be controlled and in many cases, get to such a state where they don’t bother you or affect your fertility anymore. With regular treatment, many women have regular ovulatory cycles, clear skin and maintain a healthy weight. The aim is to get your body functioning normally again, rather than ‘cure’ the disorder.
