In a 6-hospital multicenter trial, 216 patients with chronic sciatica caused by herniated disc were randomized to receive either 10 sessions of acupuncture or sham acupuncture over 4 weeks. Compared to the sham group, the acupuncture group reported a 30.8 mm decrease in VAS leg pain after treatment (P < .001), and this benefit persisted for 52 weeks of follow-up, with zero serious adverse event reports.

Meta-Analysis — 30 Clinical Trials, 2,662 Patients

A 2023 met/analysis in Frontiers in Neuroscience found 30 randomised controlled trials with 2,662 sciatica patients. Overall the total effective rate for acupuncture was significantly greater than other medical treatments (RR = 1.25, 95% CI: 1.21-1.30). The rate of recurrence was 73% lower with acupuncture (RR = 0.27) and adverse side effects were 62% lower (RR = 0.38). The authors stated that ‘acupuncture therapy is effective and safe, [and] can be considered as an appropriate alternative treatment instead of medicine.”

Meta-analyses conclude simple surgical procedures by experienced surgeons results in faster initial pain relief, but no significant difference on clinical outcome results at follow up of 1-2 years, vs. conservative treatment. Acute Sciatica After Sports Injury

A 35 year old recreational athlete: presented with sudden severe sciatic nerve pain in the aftermath of a gym work out. shooting pain, starting from the lower back, passing through the left buttock and radiating down in to the leg, with numbness affected in the foot. Unable to walk for more than 5 minutes

Diagnosis: acute qi stagnation with blood stasis in the Gall Bladder channel. Acupuncture 3 times in the first week followed by twice weekly for 3 more weeks used on top of cupping therapy over the lumbar spine and buttock region.

Full Activity Resumed in 4 Weeks — 8 Sessions Total

The acute presentation means no structural disc damage, and recovery was considered to be rapid. Patient was able to exercise moderately within 3 weeks and train at maximum intensity within 6 weeks of presentation, with no resulting episodes of L4-L5 herniation at 6 months. Same case would be expected to respond more quickly to acupuncture in standard clinical practice.

Sciatic Nerve Pain During Pregnancy

A 31 year old presentation: at 28 weeks pregnancy, experienced rising sciatica pain. Conventional options are limited even in modern pain management thanks to restrictions on drug use in pregnant women. Patient was referred for acupuncture to give some pain relief benefits, and avoid the use of analgesic medication.

Diagnosis: kidney deficiency pattern with spleen qi weakness. Gentle acupuncture at a designed set of points repeatedly twice weekly, and moxibustion at predetermined heating points where the pregnancy mandate. No herbal medicine prescribed during pregnancy as a rule.

Most cases of sciatica do turn out to be cured given appropriate treatment. Acupuncture has been established as a safe nonpharmacologic method of treatment for sciatica in pregnancy.
