Why Your Shoulder Hurts — Common Causes That Respond to Acupuncture

Shoulder pain limits daily activities, from reaching overhead to sleeping well. Shoulder injuries are common in busy Dubai clinics – from acute sports injuries to degenerative conditions – and involve the shoulder joint – the most mobile ball-and-socket joint in your body – which depends upon a complex network of rotator cuff tendons — a group of muscles and ligaments — plus the joint capsule to maintain full range of motion. When inflamed, torn, or stiff, pain and signs of reduced movement are unavoidable.

Six shoulder pain conditions respond well to our Dubai treatment protocols:

In frozen shoulder (adhesive capsulitis), the capsule of the shoulder joint thickens and adheres to itself – causing pain and restricted movement. Frozen shoulder progresses through three stages: freezing, frozen, and thawing. Improving frozen shoulder with acupuncture reduces pain and improves function by 76.4% when combined with physiotherapy for frozen shoulder.

Rotator cuff tear, tendonitis, and shoulder impingement — these rotator cuff injuries involving partial tears and impingement – may be caused by prolonged overhead activity, muscular imbalance, trauma, or ageing. A 2024 systematic review identified that acupuncture was significant in relieving pain, stiffness, and symptoms.

Chronic inflamation relating to shoulder dislocation, shoulder arthritis, and bursitis – occurs within the joint itself or bursae of fluid-filled sacs around the joint, with a significant reduction in the pain from needle insertion within 10 minutes noted for shoulder bursitis. Tendonitis and recovery from dislocation also show positive response from Beijing-trained, evidence-based acupuncture points.

TCM Diagnostic Framework

In Traditional Chinese Medicine, shoulder pain falls into distinct diagnostic patterns, each requiring a different acupuncture protocol. Wind-Cold-Damp Bi Syndrome presents as pain worsening in cold or damp weather — treated with warming needle acupuncture and moxibustion. Qi-Blood Stasis involves fixed, sharp pain at specific points following injury — treated with electroacupuncture and cupping. Liver-Kidney Deficiency manifests as chronic, dull shoulder pain in patients over 50 — addressed through constitutional acupuncture combined with herbal medicine. Our TCM consultation begins with pulse diagnosis, tongue assessment, and detailed palpation to determine your pattern.

How Acupuncture Treats Shoulder Pain — The Science Behind TCM

Acupuncture for shoulder pain works through four scientifically validated mechanisms. Locally, insertion of sterile needles promotes the release of adenosine — a natural substance that reduces inflammation. Through the spinal cord, it triggers segmental inhibition to block pain signals. Systemically, 2 Hz electroacupuncture prompts the release of your body’s most powerful natural painkillers: β-endorphin, enkephalin, and endomorphin. At the cellular level, it draws monocytes and macrophages toward inflamed tissue — releasing opioid peptides that bind to pain receptors on peripheral nerves.

As supported by a 2024 meta-analyses, acupuncture is proven to produce lasting pain relief to the shoulder joints – relief that lasts a year.

Key Acupuncture Points for Shoulder Pain

The acupoints our practitioners perform needling at depends on the diagnosis pattern your doctor or acupuncturist diagnoses for your shoulder pain. Below are the most common acupoint protocols for simple one-sided shoulder pain:

Clinical Acupoint Protocol

Local points

LI15 (Jianyu) at the anterior deltoid region improves shoulder joint qi and blood circulation. SI9 (Jianzhen) at the posterior axillary fold addresses deep rotator cuff pain. TE14 (Jianliao) targets posterior shoulder pain. GB21 (Jianjing) — located at the midpoint between the acromion and spine — reduces shoulder stiffness and muscle tension.

Distal point

ST38 (Tiaokou) on the lower leg is specifically indicated for frozen shoulder and acute adhesive capsulitis — patients move the affected shoulder during needle stimulation at this point for enhanced mobilization.

Electroacupuncture protocol

LI-11, LI-15, GB-21, and SI-9 stimulated at 5 Hz, with GB-34 (Yanglingquan) bilaterally for rapid pain control and stiffness release.

Each acupuncture treatment session at Tongren lasts 30–45 minutes. We combine needle therapy with moxibustion for cold-pattern shoulder pain and cupping therapy to release deep muscular adhesions — an integrated approach that addresses both the symptom and the root cause according to TCM principles.

Shoulder Conditions We Treat at Tongren Dubai

Frozen Shoulder (Adhesive Capsulitis)

TCM approach: Warming needle acupuncture + moxibustion + gentle Tui Na mobilization. We target the shoulder joint capsule through local acupoints (LI15, SI9, TE14) combined with the distal point ST38 (Tiaokou) — a technique validated by a 2018 systematic review for improving range of motion in adhesive capsulitis patients.

Expected timeline: 8–12 sessions over 4–6 weeks. Most patients experience noticeable pain reduction by session 3–4 and meaningful mobility improvement by session 8.

Rotator Cuff Injuries & Tendonitis

TCM approach: Electroacupuncture at the tendons of the rotator cuff combined with Tui Na deep tissue techniques. For rotator cuff tears and rotator cuff injuries where conservative treatment is appropriate, we reduce inflammation around the tendon while promoting blood flow to accelerate tissue repair.

Expected timeline: 10–15 sessions over 5–8 weeks, depending on injury severity.

Shoulder Impingement & Bursitis

TCM approach: Meridian-based acupuncture targeting the subacromial space, combined with cupping to reduce inflammation and restore pain-free overhead movement. This non-surgical treatment approach avoids the risks associated with repeated cortisone injections — and unlike shoulder arthroscopy, requires no incisions or recovery downtime.

Expected timeline: 6–10 sessions over 3–5 weeks.

Chronic Shoulder Pain & Arthritis

TCM approach: Constitutional acupuncture + herbal medicine for long-term joint health. For patients who wish to avoid shoulder replacement or prolonged orthopedic intervention, TCM provides a conservative path to manage chronic joint degeneration. We address both the local shoulder pain and the underlying Liver-Kidney Deficiency pattern common in arthritis patients, combining physical therapy principles with Traditional Chinese Medicine for sustained pain management.

Expected timeline: Ongoing maintenance, typically 2–4 sessions per month.

Acupuncture vs Conventional Shoulder Treatment — Evidence-Based Comparison

Patients in Dubai facing shoulder problems have multiple treatment paths — from physical therapy and anti-inflammatory medications to cortisone injections, shoulder arthroscopy, and shoulder replacement for severe cases. An orthopedic specialist may recommend surgery, but many shoulder conditions respond to non-surgical conservative treatment first. Here is how acupuncture compares based on published clinical data:

Factor Acupuncture Cortisone Injection Physiotherapy Arthroscopic Surgery
Pain Relief Onset 1–3 sessions 24–48 hours 2–4 weeks 6–12 weeks post-op
Long-term Efficacy ≥12 months (meta-analysis) 3–6 months then fades Variable, exercise-dependent Permanent (if successful)
Sessions/Visits 6–15 sessions Max 3–4/year 12–24 sessions 1 surgery + 3–6 months rehab
Side Effects Minimal (minor bruising) Cartilage damage risk, tendon weakening Temporary soreness Infection, stiffness, nerve damage
Invasiveness Non-invasive Minimally invasive Non-invasive Surgical (general anesthesia)
Combined Benefit 76.4% improvement with exercise Not typically combined 39.8% alone Requires post-op physio
Cost Range (Dubai) AED 250–500/session AED 500–1,500/injection AED 300–600/session AED 25,000–80,000+
Insurance Coverage Yes (BUPA, Emirates, Cigna) Yes Yes Pre-authorization required

A multicentre randomized controlled trial involving 425 patients showed that acupuncture combined with physiotherapy significantly improved shoulder function and relieved pain compared with physiotherapy alone - and decrease the need for taking analgesic drugs. For post-surgical patients who want to choose a conservative approach and avoid surgery, it compares with cortisone injection without the 3-4 times (per year) limitations and the tendons weakness.

Your Treatment Journey — What to Expect at Tongren Dubai

Visiting a Traditional Chinese Medicine clinic for the first time involves a diagnostic process different from Western orthopedic consultations. Here is exactly what happens at each stage:

01

Step 1: TCM Consultation & Diagnosis (20–30 minutes)

Your practitioner - who will have graduated from a university offering a formal TCM education programme, with multiple years clinical experience - begins by taking your pulse at three sites on each wrist, examines your tongue for internal pattern diagnosis, then palpates the shoulder joint and shoulder blade, muscles and tendons around the shoulder for evidence of certain pathology. This results in your provisional diagnosis being made according to the pattern imbalances identified (Wind-Cold-Damp, Qi & Blood stasis or constitutional deficiency.

02

Step 2: Personalized Treatment Plan

Once you are clear as to your provisional pattern diagnosis, we choose from several treatment techniques (needle acupuncture at condition specific points for shoulder joints; electroacupuncture at more problem specific points for deeper shoulder pain; moxibustion at diaphragm areas for cold presentation; cupping therapy at the shoulder blades for muscular adhesions; tui-na remedial massage for lost shoulder mobility).

03

Step 3: Acupuncture Session (30–45 minutes)

The practitioner inserts the appropriate number of fine, sterile, single use needle at the acupoints for your presentation as you report a sensation of a mild semi dull pressure, as various sensations may be experienced in the different acupoint areas (not a sharp pain). The time practitioner relaxes as the dissolving motor/immune response organises around the needles can be considered the time that the treatment is delivered. During this time, some patients require a sleep!

04

Step 4: Complementary Therapies (as needed)

Your practitioner may also include moxibustion (herbal warming treatment used close to acupoints), cupping therapy to release deep set tissue tightness, or Tui Na shoulder massage.

05

Step 5: Follow-Up & Home Care Guidance

We also offer stretching exercises and lifestyle modifications Between sessions. Initially, we begin treatment at 2 sessions a week and then reduce it to once a week as your shoulder gets better; finally, transition to maintenance care.

How many acupuncture sessions do I need for shoulder pain? +

Most conditions of shoulder pain will take between 6 and 15 treatments, 1 to 2 times a week. Frozen shoulder usually takes 8 to 12 treatments over 4 to 6 weeks. Rotator cuff injuries can take between 10 and 15 treatments. Your therapist will develop a specialized treatment program after your first Traditional Chinese Medicine consultation and modify it according to your rate of progress.

How much does acupuncture cost in Dubai? +

We do offer direct billing facility for patients insured with BUPA, Emirates, Cigna, Sukoon and Neuron. Most of the patients who have shoulder pain treatment in Dubai are covered by insurance either in part or full.

Can acupuncture treat frozen shoulder? +

Frozen shoulder (adhesive capsulitis) is the condition most amenable to treatment with acupuncture. Evidence shows that acupuncture in association with gentle exercises increases range of motion, decreases pain and stiffness compared to exercise alone. Our protocol aims at releasing the thickened capsule of the glenohumeral joint through the use of specific acupoints, including ST38 (Tiaokou), a distally effective point for adhesive capsulitis.

Is acupuncture safe? Does it hurt? +

Absolutely. All needles are sterilized and single-use disposable. The sensations you experience during this treatment are usually mild pressure and a dull ache rather than a piercing sensation. Sometimes slight bruising occurs, but usually clears within 24-48 hours.

Can Muslims receive acupuncture treatment? +

Yes, of course. We are practicing acupuncturists. Therefore, acupuncture itself is also an Islamic thing. It doesn't use any forbidden things such as drugs and other stuffs. Just clean disposable needles are used. Most of the patients who come for us are the Muslims who are living in Dubai.

What is the difference between acupuncture and dry needling? +

Acupuncture is a complete system of medicine based on the principles of Traditional Chinese Medicine and codified with over 2000 years of clinical experience. Acupuncture treats each individual patient as a whole and not just specific symptoms through the channels (meridians) utilizing diagnosis and treatment based upon the channels. Dry needling by contrast maps certain individual muscles with pain but does not encompass the full-body diagnosis and treatment within the channels. Our TCM practitioner is trained in not just channel points for shoulder conditions, but Chinese herbal medicine, moxibustion, cupping and other therapies to give greater choices of treatment than dry needling.
