Understanding Premature Ejaculation Causes: Why Men in Dubai Choose TCM

An estimated 30% of men will experience premature ejaculation during their lifetime, making it the most prevalent male sexual disorder. Of the men seeking treatment for PE in Dubai, all have tried conventional methods before and had difficulty tolerating their side effects.

SSRIs: Side Effects That Undermine the Goal

Dapoxetine and paroxetine delay ejaculation through blocking reuptake of serotonin as a neurotransmitter. Yet 25% men report nausea; decreased libido is also highly prevalent. And long term use of these drugs can cause post SSRI sexual dysfunction (PSSD), a state of permanent sexual numbness.

Numbing Creams: Both Partners Lose

Effects of topical lidocaine and prilocaine numbing creams can be felt on penetration, when reduced sensation on the penis is transmitted to the partner. Unfortunately they provide only temporary relief, temporarily taking your penis out of commission without treating your issue at its core.

PE and ED Often Coexist

It has been found that PE can often co-occur with ED – men who create a time pressure on intercourse by ejaculating earlier to avoid losing the erection actually tend to fall into the same pattern. Selective serotonin reuptake inhibitors (SSRI) can further contribute to the ED effect, and classical Chinese medicine approaches treat both disorders under the same protocol (see our ED treatments page).

TCM: A Different Mechanism

Through acupuncture and Chinese herbs, the brain’s neurotransmitter pathways are altered to remove hypersensitivity of the dorsal penile nerve, rest control over the ejaculatory process to the body’s own reflexes, and add balance to the system. Vs. using drugs to block whatever neurotransmitter activity is “in the way”, Chinese medicine finds a method to treat PE that is nerve pathway independent, balancing rather than overriding.

How Acupuncture & Herbs Work as a Natural Premature Ejaculation Treatment

Chinese medicine has proven effective with three basic types of premature ejaculation. Those who involve practitioners make their diagnosis by various cues in pulse, looking at the tongue, asking a detailed sexual history – your practitioner will select the correct Chinese pattern(s) to align your ongoing treatment.

Three PE Patterns in TCM Diagnosis

Kidney Deficiency

Loss of Jing (seminal essence) builds up from the Kidney, which is responsible for storing and monopolizing the body’s reproductive power. Ejaculation becomes far easier because there isn’t enough energy in the kidneys to “hold the gate”. Chronic fatigue, lower back soreness, dry mouth, and frequent urination are common symptoms.

Liver Damp-Heat

Liver heat generates excess fire that flickers in the pelvic region, causing the radiation of that fire an expression of the ejaculatory reflex. Lower ab soreness, hot flushes, and bitter taste in mouth are characteristic signs of this pattern. Alcohol excess and stress escalate this condition.

Heart-Spleen Deficiency

An internal worry state affects the Heart and Spleen, depleting their nourishing energy and disturbing the mind-body complexes necessary for ejaculatory mastery. Performance fear, trouble sleeping, and cold hands are typically seen in this pattern. Spirit-mind calming acupuncture points relieve this pattern.

Acupuncture Points for Premature Ejaculation

Primary Treatment Points

  • CV4 Guanyuan Mon3 (RR7) Un11 To: Tonify Kidney qi strengthen the lower Dantian and cement your ejaculatory function.
  • SP6 Sanyinjiao Liuying-in (UB22) and Four Gates – regulate the three diagnostic patterns by harmonizing the three Jing vessels
  • BL23 Shenshu Tonify the Kidney
  • HT7 Shenmen Calm the spirit
  • LR 3 Taichong Clear Liver Heat
  • ST36 Zusanli – General Qi builder; tonifies the Spleen to support nourishment of the reproductive system

New research to be published in 2024 provides insights into the bioelectric mechanisms involved: needling at these points lowers sensitivity of the dorsal penile nerve, affects the 5-HT neurotransmitter pathways in the brain, and inhibits the rhythmic contractions of the bulbospongiosus muscle. Combined, these mechanisms account for the delayed ejaculation times seen in sham controlled clinical trials of acupuncture at these points. Every one of these findings reflects measurable physiological change, not placebo response.

TCM vs Premature Ejaculation Medicine: Comparing Early Ejaculation Treatment Options

All means of treating premature ejaculation are nuanced, the best matching your pattern diagnosis. This tool allows you to evaluate published data on clinical outcomes and choose which of these ways of treating yours makes most sense to you and your lifestyle.

Treatment Parameter TCM (Acupuncture + Herbs) SSRIs (Dapoxetine / Paroxetine) Behavioral (Squeeze / Stop-Start) Topical (Lidocaine Spray / Cream)
Mechanism Serotonin regulation + nerve sensitivity reduction + Qi balance Delays serotonin reuptake in CNS Psychological reconditioning of ejaculatory reflex Local nerve numbing via anesthetic
Onset 3–4 weeks (gradual, cumulative) 1–3 hours (on-demand dosing) 4–12 weeks with consistent practice Immediate (15–20 min before intercourse)
Side Effects Minimal — mild needle sensation, occasional light bruising Nausea (25%), dizziness, libido loss, PSSD risk None Reduced sensation for both partners, possible allergic reaction
Duration of Results Root-cause treatment — results persist after course ends Must continue medication; PE returns when stopped Requires ongoing practice; skills can fade Per-use only; no lasting effect
Cost (AED) 2,400–6,600 per course 100–400/month (ongoing indefinitely) 400–900/session × 6–12 sessions 50–200/month (ongoing)
Compatibility Combines safely with all other treatments Contraindicated with MAOIs, some SSRIs, and certain medications Combines with all treatments Combines with all treatments

TCM + SSRIs Combination:

Emerging evidence shows that combining acupuncture with low-dose SSRIs produces enhanced outcomes compared to either treatment alone. Some patients begin with combination therapy, then taper off the selective serotonin reuptake inhibitor as acupuncture restores natural ejaculatory control. Discuss this option with your prescribing doctor and our practitioners.

PSSD Warning — Worth Knowing Before Starting SSRIs:

Post-SSRI Sexual Dysfunction (PSSD) is a recognized condition where sexual side effects — reduced genital sensation, difficulty with orgasm, lower libido — persist after stopping SSRI medication. The European Medicines Agency issued a safety warning about PSSD in 2019. While it affects a minority of users, the condition has no established cure. For men taking antidepressant medication off-label specifically to delay ejaculation, this risk deserves careful consideration.

Clinical Evidence — Research on Acupuncture for Premature Ejaculation

There is a growing number of peer-reviewed published research articles on the efficacy of acupuncture for premature ejaculation, including the articles that follow. We aim to be transparent about what those articles tell us, and what remains to be clarified.

RCT — European Urology

Sunay et al. — Acupuncture vs Paroxetine for PE

This randomized trial compared acupuncture to paroxetine (an SSRI) and placebo in the treatment of premature ejaculation. Patients receiving acupuncture were given an IELT (intravaginal ejaculation latency time) delay significantly superior to placebo, and comparable to paroxetine, with an increased number of delayed ejaculation reported post-treatment.

Systematic Review — 2023

Zhang et al. — Acupuncture for PE: Systematic Review & Meta-Analysis

Acupuncture for managing premature ejaculation: a systematic review. Sexual Medicine Reviews (academic.oup.com). 2023. This online systematic review looked at multiple published clinical trials with various acupuncture protocols for premature ejaculation. Overall, patients receiving acupuncture responded significantly better regarding IELT and self-identified sexual satisfaction measures.

Clinical Study — 2019

Li et al. — TCM for Premature Ejaculation

Investigating the effectiveness of traditional Chinese medicine treatments for early ejaculation: a 2019 systematic review (PMC6504282). The authors report that combined acupuncture and herbal treatments for PE yielded favorable stability and safety and proved more effective than the medication-only control group.

Mechanism Study — 2024

Wang et al. — Mechanism of Acupuncture in PE Treatment

Published in 2024 (PMC11636054), this study revealed how specific bioelectric actions of acupuncture including desensitisation of the dorsal penile nerve, serotonergic 5-HT pathways, and contractions of the bulbospongiosus muscle contributed to the delayed ejaculatory reflex seen in earlier clinical trials for acupuncture.

Premature Ejaculation Treatment Dubai: Your Healing Journey at Tong Ren Tang

Every case of premature ejaculation is different. Your local practitioner in our Dubai Healthcare City clinic, or in Jumeirah, will carefully progress you through a structured treatment template that, over time, delivers true, demonstrable change.

1

Confidential Consultation (45 Minutes)

A detailed sexual health history will be compiled by your practitioner, which will include estimates of ejaculation latency, your relationship circumstances and your current medication load. Traditional Chinese diagnostics – including pulse diagnosis, tongue diagnosis and pattern interrogation – will identify your fundamental imbalance. Your information is kept in the strictest confidence. This is a truly private clinical setting, devoid of judgement and expectation.

2

Pattern Identification

Your practitioner will then pinpoint which of the three PE syndromes sits beneath your condition – Kidney Deficiency, Liver Damp-Heat or Heart-Spleen Deficiency. Many of our men have a mixture of pattern causes. This diagnostic formulation guides your own unique treatment protocol – the choice of acupuncture points, herbal composition and any additional therapy.

3

Treatment Course — 8 to 12 Sessions

Treatment in Dubai Healthcare City takes place twice weekly for 4-6 weeks, with acupuncture (25-35 minutes – with needles retained), often followed by either cupping or moxibustion, depending upon your diagnosis. Custom herbal formulas are prepared as concentrated granules, so that you take them daily in between sessions. Most men notice significant shifts in pelvic control by the third or fourth session.

4

IELT Assessment — Measuring Your Progress

Your ejaculation latency time and your sexual gratification are checked in the three treatment sessions scheduled at the conclusion of the first six weeks of care. Your results are now quantitative, providing hard evidence of your progress. If progress stalls, your acupuncture point combinations or herbal formula are tweaked accordingly. Your treatment is flexible, responding to your individual body’s response.

5

Consolidation & Lifestyle Integration

The final 2-4 treatments constitute your consolidation phase, and involve live instructions by your practitioner in Kegel strengthening exercises and pelvic focus control techniques, and take place on a weekly basis. Diligent attention to your Kegel mastery allows you to raise levels of capacity well beyond the number of treatment sessions. Finally, detailed dietary guidelines in accordance with your pattern you are handed, so that your long-term control is maintained far beyond your duration of care.

Frequently Asked Questions — PE & TCM Treatment in Dubai

How effective is acupuncture for premature ejaculation?

One European Urology placebo-controlled trial demonstrated that the acupuncture group experienced a measurably stronger ejaculation-delaying response than the placebo group. Patients experienced objective increases in IELT and subjective increases in satisfaction scores throughout the trial period.

How many sessions do I need?

A standard premature ejaculation treatment course consists of 8-12 acupuncture sessions, usually twice weekly over a 4-6 week course of therapy. Many men notice a clinical effect within the first 3-4 sessions. Your practitioner will individualize your course of therapy based upon the results of your self-measured IELT and self-reported improvements throughout the course of therapy. A 2-4 session weekly consolidation course usually follow the original course of therapy.

Can I keep taking my SSRI during TCM treatment?

Yes- acupuncture and herbal Chinese medicine are safe to use alongside the SSRI medications such as dapoxetine or paroxetine (paxil). Many men continue their TCM treatment while still taking the SSRI, at first working with their prescribing doctor to wean off the chemical, then reducing the SSRI dose as the TCM treatment takes effect. Our practitioners are happy to work in coordination with your urologist or general practitioner in Dubai. Never stop any prescribed medication without consulting your doctor first.

What causes premature ejaculation from a TCM perspective?

There are three main patterns of the TCM pathology of premature ejaculation. Kidney Deficiency results in the inability of the Shen to consolidate the Jing, causing a weak “gate” that fails to control the ejaculation reflex. Liver Damp-Heat creates excess nervous system excitability that causes reflex acceleration. Heart-Spleen Deficiency- caused by worry, overthinking, lack of rest- depletes the Shen and Jing, preventing mind and body coordination necessary for really successful intercourse. Most men suffer from a combination of these patterns.

How much does PE treatment cost?

A full course of TCM therapy, including Singapore with herbal prescriptions, costs between AED 2,400 and AED 6,600. The total depends upon the number of acupuncture sessions recommended for the individual as well as the number and types of herbal formulas prescribed. Your first office visit assessment lasts 45 minutes and determines the total course of therapy.

Can you treat PE and ED together?

Definitely. There is a high rate of coexistence of premature ejaculation with erectile dysfunction and TCM protocols treat both entities at the same time. The same acupuncture points, herbal formulas, and nutritional recommendations that improve the ejaculatory threshold contribute to increased genital sensation and erection quality. For further information see our erectile dysfunction treatment page.

Are there side effects from acupuncture PE treatment?

These adverse events are temporary and very rare. Some patients experience a dull ache, warmth or tingling at the needle sites during the course of a session. This is called “de qi” and it indicates the acupuncturist has fully engammed the specific body meridian at that point with qi. Occasional superficial bruising at an acupuncture point usually fades within 24 hours and there is no nausea, no diminished desire, no addiction potential, and no risk of the permanent anorgasmic sexual dysfunction which SSRI’s carry.

What is PSSD and should I worry about SSRIs?

Post-SSRI Sexual Dysfunction (PSSD) is a phenomenon where the sexual side effects- lessened genital sensation, impaired ability to respond to orgasmic stimuli, diminished libido- persist long after cessation of the SSRI medication. Official acknowledgement of PSSD occurred by the European Medicines Agency in 2019. Contrary to the common perception that this side effect is very rare, PSSD can also occur in conjunction with SSRI antidepressant treatment prescribed specifically to delay ejaculation. There is no proven remedy for this complication, but non-drug therapies such as acupuncture and Chinese herbal medicine are effective and do not carry this risk.

What is the squeeze technique and does TCM replace it?

The squeeze technique involves firmly squeezing the glans penis at the time of ejaculation, and removing the stimulation until the sensation sensation reduces. It is a common behavioral therapy used with stop-start training. TCM does not displace it – acupuncture helps prevent the physiological burden of PE (nerve senstitivity, serotonin regulations) and the behavioral therapy provides psycological self-control. The combination of these two techniques generally yields better results than either alone.
