Clinical Evidence — Why Acupuncture Works for Disc Herniation

Lumbar disc herniation acupuncture is not primitive. It is evidence-based therapy evidenced by a growing history of well-published controlled clinical trials.

The World Health Organization's report in 2002 acknowledged the efficacy of acupuncture for twenty-eight disease states from low back pain, a benchmark that subsequent research continues to validate. A 2024 selected RCT in Frontiers in Medicine demonstrated that combined treatment with acupuncture and spinal manipulation was statistically superior to traction treatment in pain scores and total effective rate in patients with prolapsed lumbar disc.

A 2025 large-scale network meta-analysis in PMC ranked acupuncture against including pharmacologic medicine, physiotherapies, and other management options in treatment of sciatica secondary to lumbar disc protrusion and confirmed it as an effective modality.

How Electroacupuncture Works at the Disc Level

Electroacupuncture preferentially releases endogenous opioids- beta-endorphin, enkephalin, dynorphin- by stimulating afferent nerves. Acupuncture at the tissue level decreases peripheral inflammatory cytokines such as Il-6 and TNF-alpha- the same mediators that are increased by herniated disc tissue. In a case report published by Karger,Electroacupuncture facilitated spontaneous resorption of herniated disc tissue-verification of reduced disc protrusion was obtained by MRI without surgery. Acupuncture has been subsequently been added to the traditional low back pain clinical practice guideline of Hong Kong.

At the clinical level, the significance of this double-and clinically dual-mechanism is: a. C-fiber afferent fibers stimulated with 2 Hz frequency component of electroacupuncture cause systemic beta-endorphin and enkephalin secretion in the hypothalamus and pituitary resulting in pain relief that has a duration of hours after the end of the session and; b. A-delta fiber afferent fibers stimulated with the 100Hz component of the electroacupuncture causes even greater localized dynorphin release in the lumbar spinal cord dorsal horn (owing to fiber scarcity) where the herniated disc has pinched the nerve causing the pain. This directly modulates the dorsal root ganglion segment receiving the compression pressure to effect functional resorption of the protrusion. This dorsal root modulation reduces inflammation and pain in the peripheral tissues corresponding to the compressed segment.

These results are controlled, peer-reviewed clinical research articles originating from international institutions. There is no guarantee of individual patient efficacy, but based on this evidence, it is simply incorrect to state that acupuncture for disc herniation is not evidence-based.
